Please read these Terms and Conditions (“Terms”, “Agreement”) carefully before using the www.choicepharmd.com website and related services (the “Service”) operated by Choice PharmD (“Company,” “we,” “our,” or “us”).
By accessing or using our Service, you agree to be bound by these Terms. If you do not agree with any part, you may not use the Service.

​Choice PharmD provides pharmacist-led clinical interpretation to support providers in understanding PCR results and selecting appropriate antimicrobial therapy.

This platform is intended for users who are 18 or older. If you are under 18, you are not permitted to use this service.
You can cancel the SMS service at any time. Just text "STOP" to 904-875-9058. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.
If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at info@choicepharmd.com.
Carriers are not liable for delayed or undelivered messages
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive messages only when you opted in to receive confirmations of your appointment or updates. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
