How Choice PharmD Interpretation Services Work: Frequently Asked Questions

Clinical pharmacists reviewing PCR results and therapeutic guidance data in a molecular lab.

Choice PharmD interpretation services help bridge the gap between molecular diagnostics and clinical decision-making. Once PCR technology identifies pathogens and resistance genes, a clinical pharmacist reviews the data, applies evidence-based reasoning, and produces concise therapeutic options tailored to the patient. The final Choice PharmD report integrates directly into the laboratory information system so clinicians see detection results and recommended therapy side-by-side.

General Questions for Lab Directors

What are treatment considerations based on?

Treatment recommendations are based on aggregate of data, including clinical practice guidelines (IDSA, CDC, WHO, AAP), primary and secondary literature (randomized controlled trials, meta-analyses, PubMed), and tertiary references (Sanford Guide, Micromedex, Clinical Pharmacology).

Which panels are supported?

We support a wide range of PCR panels, including respiratory, UTI, GI, wound, STI, women’s health, and nail.

What is included in a Choice PharmD report?

Each PharmD Report includes an explanation of detected pathogens and resistance genes, clinical interpretations of results, and clear therapeutic guidance written in practical, provider-friendly language. The report is reviewed by a clinical pharmacist and appended to your lab’s existing report for a complete, unified output.

What is included in a Choice PharmD Stewardship Report?

Our Stewardship Reports provide a detailed breakdown of your lab’s detection rates which are compared to our network of labs. Additionally, clinic stewardship reports may be provided upon request.

Do you use algorithms to generate treatment recommendations?

We use advanced clinical algorithms developed by pharmacists and physicians, but every single report is manually reviewed by a PharmD before delivery. This ensures accuracy, efficiency, and clinical soundness.

Questions About Clinical Interpretation

Can clinicians speak directly with a pharmacist if they have questions?

Yes. Each report includes a QR code that connects providers to a live, trained pharmacist who can answer case-specific questions or clarify therapeutic recommendations.

What are sensitivity tables and how are they used?

Sensitivity tables are an optional add-on that provides a comprehensive list of medications with antimicrobial activity for  detected pathogens, taking into account resistance genes detected. This feature provides additional treatment options in the case the patient may require a medication not listed in the table (i.e. IV options due to severity of disease).

Do you take into account allergies, renal function, patient age, concomitant medications, etc?

Yes, our medication tables include dosage adjustments tailored to patient age and renal function. When additional patient information is available—such as allergies, concomitant medications, and other relevant details—we incorporate it into the process to ensure the most accurate and personalized medication regimens for treatment.

LIS & Integration Questions

How is the PharmD report combined with a laboratory report?

The PharmD report is transmitted as a PDF and is typically combined with the lab report resulting in a single PDF. This creates a comprehensive report, featuring laboratory results along with therapeutic guidance. The PharmD report can be positioned either at the beginning or the end of the lab report, based on preference.

Does Choice PharmD integrate with our lab’s LIS?

Yes, we establish a robust HL7 integration with your lab's Laboratory Information System (LIS) to ensure seamless and efficient data transmission. This integration allows for the automated exchange of information, streamlining workflows and reducing the risk of errors. By facilitating smooth communication between your lab and our software, we enhance the overall efficiency and accuracy of the reporting process.

What is the workflow for PharmD reporting and lab reporting?

After your lab completes sample results, the data will be transmitted to Choice PharmD through an HL7 message. Our pharmacists will review the information and prepare a detailed PharmD report for each result. A PDF will then be securely sent back to your Laboratory Information System (LIS) via Secure File Transfer Protocol (SFTP) and seamlessly appended to your lab’s results.

Billing, Workflow & Turnaround Time

What is the turnaround time for completion of PharmD reporting?

Following submission of lab results, our average turnaround time (TAT) is approximately 10 minutes during business hours and 15 minutes during evening hours. PharmD reporting is guaranteed to be completed within 1 hour.

How are Choice PharmD services billed?

We charge the laboratory a flat fee per PharmD report. There are no onboarding costs, consultative fees, or other hidden charges.

Are Choice PharmD sensitivity tables included?

Sensitivity tables are available upon request as supplement to the PharmD report. They can be configured for an entire lab or added selectively for specific facilities.

Compliance, Guidelines and Clinical Stewardship

How do Choice PharmD reports support antimicrobial stewardship?

By providing pathogen-specific therapy guidance, Choice PharmD helps clinicians avoid overtreatment, reduce broad-spectrum use, and align with national stewardship goals.

Are Choice PharmD reports consistent with treatment guidelines?

Yes. Pharmacists incorporate guidance from IDSA, CDC, AAP, WHO, ACOG, and other evidence-based sources.

Summary

Choice PharmD interpretation services transform raw molecular data into guidance clinicians can act on immediately. Instead of expecting providers to decipher pathogens and resistance genes alone, labs can deliver a pharmacist-generated report that clarifies significance, recommends evidence-based therapy, and supports antimicrobial stewardship. As resistant infections rise, having Choice PharmD at the interpretation layer becomes a meaningful clinical advantage.
